That couldn't be hard to fix.  Likely only a couple lines
of Python code.  But would you want to?
Look at some large organizations.  For example

  John.Doe@laafb.af.mil 
  John.Doe@wpafb.af.mil

It is very likey there are two "John Does" in the US Air Force,
one here at the LA Air Force Base and the other at Wright
Patterson AFB in Ohio.  

And what about places like the UK were many people have the
same second level domain?

I think what is needed is a way to handle it on a case by case
basis.
